Freeport Health Centre – Communication and Technology Clinic provides multidisciplinary assessment services to adults 19 years and over with communication and writing needs.  The clinic also authorizes Assistive Devices Program (ADP) financial assistance for eligible individuals.  For further information regarding this clinic, call:  519-749-4300

Waterloo Region Social Services Department Community Outreach Program provides information on and referral to available community services for individual support that may require food, clothing, shelter, transportation, child care, recreation, employment, education or counseling services.  For further information , call:  519-883-2290 or visit their website at:  www.region.waterloo.on.ca

 

Waterloo Wellington Community Care Access Centre provides information on health and community based services and referral to community supports and day programs.   These services include: assessment for visiting health and support services which may include nursing, physiotherapy, occupational therapy, speech language pathology, nutrition counseling, social work, personal support and respite services.  The CCAC also provides long term care placement and respite services.   Assessment service may be accessed via SELF or GP referral .  For further information, please call:  519-748-2222 or visit their website:  www.ccac-ont.ca
